Congratulations, you're pregnant! We offer comprehensive prenatal care including care for high risk pregnancies.
Patient Registration
For your convenience, Four Seasons Ob/Gyn has posted "New Patient Registration" forms. We ask that you complete and fax back to our office to allow our staff the chance to Pre-Register you in our database. This process will alleviate any extra time in your busy schedule.
Our front office personnel will input all demographic information including any insurance plans. Once this step has been completed, we will contact you for your appointment time. Please be sure and indicate if you have a specific time or day preference.
The day of your appointment, we ask that you arrive at least 5 minutes early to ensure that all information has been correctly documented. Please be sure and bring the following information with you:
- Current Insurance ID(s)
- Drivers License
Office Policy: Please provide a 24 hour notice for cancellation or rescheduled appointments. Missed appointments may result in a $25 fee.

Fee Planning
One of our professional staff members will meet with our obstetrical and surgical patients to discuss your financial policy and obligations.
Electronic Medical Records
Computerized medical record, billing, prescription writing, and lab results. Your chart is never lost, misplaced or destroyed. Insurance claims are filed efficiently and coverage benefits are known instantaenously. This means a more streamlined office and less paperwork.
